Richmond Behavioral Health is seeking to hire a skilled and qualified Care Navigator to support our Regional Services Team! The selected individual will be responsible for providing follow up, continuity of care, and/or community resources and linkages for individuals experiencing a behavioral health crisis; responsible for contacting individuals who have an expressed needs for additional community services and/or resources, therefore assisting the crisis call counselor to quickly move on to serving other individuals in need of more immediate crisis triage and mobile crisis service connections; responsible for exploring the individual’s interest in seeking further services and/or other community connections in a person-centered, recovery-oriented approach, engaging a full array of available options (CSB services, private behavioral health providers, peer services, advocacy organizations, and opportunities for community connection). This position is responsible for collaborating with all Region 4 Community Services Boards and will engage with multiple internal and external stakeholders. Work is performed under the moderate supervision of the assigned Program Supervisor.

Essential Functions

Retroactively reviews all case dispositions from the Regional Crisis Call Center and provides follow up with callers of Regional Crisis Call Center denoted as in need of additional service supports and/or community resources.

Provides follow-up with callers in need of additional service supports

Navigates active cases to local CSB same-day access for a warm hand off to ensure individuals receive timely and effective crisis services

Serves as a point of contact for callers electing to re-engage after conclusion of crisis event

Documents and maintains data in accordance with regional fiscal agent, regional programs office, DBHDS, and other stakeholder requirements.

Develops and maintains a strong partnership with Regional Crisis Call Center vendor to ensure consistency in policies, procedures, and care navigation for callers.
Works with direct supervisor to to identify any service area gaps to engage internal and external stakeholders to provide robust menu of community services/resources for individuals in crisis and post-crisis.

Actively participates in committees/workgroups as assigned

Completes documentation in compliance with all program requirements.
